This is for all year groups.

You will note that consent can only be given online by going to https://childconsent.co.uk/kchft and completing the online registration with the NHS.  We at Westlands Primary School cannot make bookings on your behalf, and only those who have registered by no later than Friday 29th October will be given the vaccination. 

Anybody who hasn't completed the online registration before 29th October will need to call the NHS on 0300 123 5205 to book their child into a catch-up clinic at a later date if they wish for their child to receive the vaccination.
